Contractions started one hour after I stepped off the plane from Oregon into California on April 4. Kalista was radiant, beautiful! She hugged me saying, "Sophie and I waited for you! Every day I told her, 'Your Gramsie is coming!' Mama, now we can have our baby!" She told me she had also been talking to her baby about the "pain" of birth. She had been at the three-day birth of her niece, Ariel Elizabeth. Like all new mothers, the stories of pain in childbirth frightened her. She wondered if she would feel tortured. "Sophie, let's work together and make this as easy as possible on both of us." She dreamt that her baby just appeared between her legs and took that as a sign that Sophie was listening to her. My favorite dream during her pregnancy was one where she was holding her baby face to face, teaching her all different kinds of ways to laugh. "Mama, she was doing it! She was making different laughing sounds just like I was making! You and Trace were there but you were in the background. Sophie and I were in our own little world…just laughing!" Those dreams became part of my power prayers, feeding that strong, happy independent energy that can be the mystery and magic of an instinctually powerful birth.

We were so excited to be together. We grabbed our cameras and went to Balboa Park to take the end-of-pregnancy photos that were another one of Kalista's dreams and desires. Kalista labored irregularly and lightly through the next day. She planned a dinner party the night of the 5th, in hopes of hanging a few last minute decorations. The unplanned enthusiasm resulted in many family and friends listening to the baby with my fetoscope, Kalista smiling and open-hearted.

The morning of the 6th Kalista called with signs of blessed show. "It can still be days, right?" she questioned me. "Oh yes, it can be days," I agreed, not wanting to bring undo attention to the increasing work I could see happening in her body. She phoned her support midwives to share the news. She wanted to be deep in nature so we went to hike Mission Gorge. Although she hiked ahead of the seven-woman group, I noticed the contractions were changing, coming a bit more frequently, lasting a bit longer and moving into her back.

That night Kalista came to dinner at her sister Amory's, where she was surrounded by love and family and ease. Sequoia Marie, her 6-year-old niece said, "Gramsie, we drove all the way from Missouri to help Auntie Ka. We drove the furtherest didn't we?" Kalista would take a few bites of tofu and veggies and then hop up and bend over while one of her sisters gave counter pressure to her hips or back. Then she would smile, sigh and sit back down to have a few more bites of yummies. "This could all just stop and wait for days, right?" Kalista asked. "Oh yes, it could…" I agreed, not wanting to bring undo attention to the now established labor. I had studied Kalista and her husband, Trace, closely during the prenatal period and my instinct told me that Kalista was going to do her best work with privacy and a quiet environment. "Like the labor of any creature," I said to her, "it can stop and start until they are safe in their nest and it is time. For you, it is time to be back in your nest and with Trace." An hour after sundown she drove herself back home in a growing labor.

High Level Meetings with Dr. Berwick, Top Medicaid Administrator

In June, the MAMA Campaign had two unique high-level opportunities to provide recommendations to Dr. Donald Berwick, Administrator of the Centers for Medicare and Medicaid Services (CMS) and visionary national health care leader.

Mary Lawlor was invited by Dr. Berwick to represent the MAMA Coalition on June 2nd at a symposium for 40 national health care leaders at CMS. Dr. Berwick spoke ardently of the vast resources that the U.S. spends on perinatal care while our outcomes worsen every year. Under his leadership, CMS is seeking advice on how to improve this dire situation, and CPMs were invited to the table.

On June 8th, Mary Lawlor and Ellie Daniels from MAMA, along with Campaign consultants Billy Wynne and Katie Pahner from Health Policy Source and Dr. Jeff Thompson, Chief Medical Officer of Washington State, were privileged to have a personal meeting with Dr. Berwick and four members of his staff. MAMA had the opportunity to recommended that CMS focus resources on the needs of the majority of childbearing women and infants who are low-risk; increase access to CPMs and birth centers to assure appropriate care for this population that too often suffers the consequences of inappropriate interventions; and include CPMs and birth centers in performance measurement, data reporting and payment reform initiatives.

CPMs Specified in CMS Guidance Regarding Birth Center Provisions

MAMA has met several times over the last year with staff at CMS to support implementation of the birth center provisions in the Affordable Care Act (ACA) as they apply to CPMs. MAMA is pleased that the recently released CMS State Plan Amendment template, containing implementation guidance to state Medicaid offices, specifies that Medicaid reimbursement of birth center provider fees applies to CPMs, a request that MAMA had made to CMS staff.

MAMA Stays Focused

While Congress wrestles with the budget deficit and the debt ceiling, while much of the country struggles with record-breaking temperatures, the MAMA Coalition is more committed than ever to increasing women's access to CPM services. We continue our steady office-by-office approach to building Congressional support for our CPM bill, HR 1054, including focused outreach to Republican offices, seeking bi-partisan sponsorship for the CPM bill.

The MAMA Campaign is growing and expanding. We now have an upgraded administrative system, a new data base platform, and recently welcomed two new staff members: Tamara Wrenn, Administrative Assistant and Farida Sharalam, Technology Assistant. We are on the move - visit us at www.mamacampaign.org!

This Month's Feature: Returning from the War Zone

Over 2 million U.S. troops have deployed to the conflicts in Iraq and Afghanistan in the past decade. The experiences they have while in the military and during deployment stay with them for the remainder of their lives. They also face many challenges, witnessing the death or injury of friends or other military personnel, civilians, or enemy combatants. Reunion can be a time of considerable stress for both troops and family too. Some service members say that coming home may actually be harder than going to war.

Returning from War

In order to get through homecoming as smoothly as possible, it is helpful to know what to expect during reintegration and the kinds of issues you, or someone you know, might face. Learn more from the Returning from the War Zone Guides. These two guides, one for troops and one for families, provide information to help smooth the reintegration process. Learn what to expect as part of normal reintegration, as well as how to tell if problems are in need of professional attention.

Military Families

Both troops and their families need to readjust and understand things before, during, and following deployment. Deployment stress affects families and here is some additional information to help.

All of those who return from deployment to war are affected, but only a few go on to develop more serious mental health issues like PTSD. Each conflict offers new challenges. Life Stressors

Life stressors such as financial problems, relationship conflict and lack of employment choices, can add to readjustment difficulties. Self Help and Coping

Read about some active ways to cope with traumatic stress, including tips for coping with specific PTSD symptoms. Also learn about negative coping strategies that you should avoid. Android and iPhone users can download the free mobile app PTSD Coach for more coping suggestions. Veterans can also speak directly with another combat Veteran by calling the 24/7 Veteran Combat Call Center: 1-877-927-8387 (WAR-VETS).

Part of self help involves getting assistance when it is called for. Be sure to know how to Discuss Trauma and PTSD with Your Doctor and know where to get care. Each VA medical center and many VA clinics provide PTSD care. VA Medical Centers and Vet Centers are listed in the phone book or can be found online in the VA Facility Locator. In the Government pages of the phone book, look under "United States Government Offices." Then look for "Veterans Affairs, Dept of." In that section, look under "Medical Care" and "Vet Centers - Counseling and Guidance."

You can also find a VA PTSD Program near you. These VA programs specialize in PTSD treatment including outpatient and residential programs.

For Providers and Researchers

Specific Types of Providers

Chaplains, law officials, primary care and OB/GYN doctors, and journalists can check out our new resources targeted to specific types of providers. This section includes a course on Military Culture to help providers understand this subculture. You can also get a glimpse of what it was like to be deployed to Iraq from the PTSD 101 feature: Iraq Never Leaves Us. Marine Reservist Bob Page shares his story to provide the viewer with an "up-close and personal" view of "What's it like over there?" in the combat theater of Iraq via his personal photos.

Assessing and Treating Veterans

To screen patients for PTSD, use the PC-PTSD Screen or any of several other PTSD screening tools. Screens are brief questionnaires that may identify people who are more likely to have PTSD. A positive response to the screen does not necessarily indicate that a patient has PTSD. However, a positive response does indicate that a patient may have PTSD or trauma-related problems, and further investigation of trauma symptoms by a mental health professional is warranted.

There are many other more detailed assessments that can be used. The Deployment Risk and Resiliency Inventory was developed specifically for use with military samples. This suite of measures includes 14 relevant factors.

All of the treatment resources you find on www.ptsd.va.gov are applicable to Veterans as well as other traumatized individuals. Also be sure to see the materials specific to treating Veterans including the Iraq War Clinician Guide. VA and DoD recently revised the PTSD Clinical Practice Guidelines to provide the best guidance on how to work with returning service members who develop PTSD.

Provider Training

Over the next year a new PTSD 101 series is being released. This group of courses will cover the revised VA/DoD PTSD Clinical Practice Guidelines (CPG) and will offer free CE's. Take the first course in the CPG series now: Recommendations for Acute Stress Management and Prevention of PTSD.

Also, be sure to sign up for the Military Health System Learning which is for DoD, VA and other providers.

National Center for PTSD Research Highlights

Research is ongoing to help us better understand and help those who are dealing with challenges following deployment, as well as following other trauma. Our Behavioral Science Division is testing PTSD-Focused Relationship Enhancement Therapy, a relationship enhancement group intervention for married or partnered Operation Enduring Freedom (OEF)/Operation Iraqi Freedom (OIF) Veterans and their partners to prevent the perpetration of intimate partner aggression. This intervention incorporates components of several interventions for these two problems and targets mechanisms implicated in the PTSD-intimate partner aggression association.

Dr. Matthew Friedman, Executive Director of the National Center for PTSD, is a finalist for the prestigious SAMMY Award for his pioneering work on PTSD and trauma. He is among an array of incredible civil servants whose contributions have earned them accolades and advanced their field of study. We commend all of these dedicated individuals. The winners are decided September 15, 2011 in a ceremony in Washington, DC honoring all the finalists.

VA Launches Childcare Pilot

Secretary of Veterans Affairs Eric K. Shinseki announced the launch of free, drop-in childcare service centers at three VA medical centers. The pilot centers are part of VA's continuing effort to improve access to health care for eligible Veterans, particularly the growing number of women Veterans. All the pilot childcare centers will be operated onsite by licensed childcare providers. Drop-in services are offered free to Veterans who are eligible for VA care and visiting a facility for an appointment.

VA-Army Agreement Helping Transform Health Care for Patients

The Department of Veterans Affairs (VA) and Department of the Army are working together to provide cutting-edge simulation-based training for medical staff to enhance the quality of patient care at VA medical facilities across the Nation. The SimLEARN Program was established by VA to develop national simulation-based clinical training throughout its more than 150 medical centers around the Nation. Welcome to Volume 25, Issue 2 of the Journal. We have three very intense and thought provoking articles from international authors in this issue. The strengthening of international connections can only add to our knowledge and move the field of prenatal and perinatal psychology and health forward as we enter the 2nd decade of the 21st century, so we are very grateful for these contributions.

Our opening article is from Loredana Cena and Antonio Imbasciati of the University of Brescia in Italy. They bring us a history and description of Perinatal Clinical Psychology and how it may be applied in clinical settings. This history includes connections with and the development of psychoanalysis and child psychology through the last century or so. A scan through the English translations in the reference list will give you an indication of the richness of prenatal and perinatal psychology literature that is not yet available in English.

Next we have Christof Plothe from Germany with an extensive literature search supporting his hypothesis that there may be a connection between the use of synthetic oxytocin in birth and the rise in autism. Those of you who were fortunate enough to attend his session at the Fall Congress know his passion for the topic and will, along with the rest of us who did not have that opportunity, appreciate having this material in writing for your ready reference. The literature search alone makes this an article worth saving.

Finally, from Canada, Dr. Philip Nye returns to our pages after a long absence. His articles, in collaboration with other researchers from the University of British Columbia, were published in the Fall and Winter, 1993 editions of what was then called the Pre-and Perintal Psychology Journal. This time, in collaboration with Claudia Sheils and Marek Gajowy, he again addresses the controversial topic of abortion, presenting evidence of post abortion survivor syndrome (PASS) in individuals whose mothers have had abortions. This is a very emotional topic and will generate some discussion of the validity of the research. However, it is important that this issue be openly discussed and the possibilities explored. In that spirit, we welcome Dr. Nye and his colleagues to these pages.

Cord Care

One simple intervention that has shown promise is cleaning the cord with chlorhexidine. (In developed countries, several decades of using binders and many years of cleaning the cord with alcohol have been found to create problems.) A study in southern Nepal found that babies whose cords were cleaned with chlorhexidine, particularly within 24 hours after birth, had drastically reduced rates of severe infection (75% lower) and death (24% lower).

Shoulder Dystocia

Shoulder dystocia is one of those complications that can easily lead to the death of a baby and has taken many homebirth midwives out of practice, either through direct experience, or due to the witch hunt in their state. It is important that you as a midwife know everything you can because different techniques work for a variety of situations. Make your list of do's and don'ts for shoulder dystocia long and thorough.

Since I have written quite a bit on shoulder dystocia in the past, I wanted to share new voices in this editorial and asked the mothers and midwives on my Facebook page to share their stories and wisdom. (The names of the first two moms have been withheld to protect their privacy.) I hope these ideas are of help to you.

Mom One: My second child had shoulder dystocia. My midwife told me I had to stay flat on my back, with my knees as high as I could get them (McRobert's). I had told her prenatally that I wanted to try the Gaskin Maneuver if a dystocia happened. I was very upset that I was told that I must stay on my back and didn't have the presence of mind to say, Screw that, I'm going on hands and knees.

She said gravity would help pull him under my pubic bone. I pushed, she pulled, my tailbone cracked (my mom even heard it across the room) and then he came out. He had difficulty breathing for the first hour, I think because they immediately said, "He's not breathing," cut his cord and took him across the room to give oxygen. I don't think his breathing had anything to do with the dystocia, or the cord around his neck, which they pulled over his head before his shoulders were born. I think it had to do with them depriving him of a large portion of his blood supply.

Radiation and Gender

A new study concludes that "the leakage of radioactive elements from nuclear tests and power plant accidents in the last 50 years" has been a cause of fewer female births worldwide.

A larger gender gap was reported in the United States and in European Countries after atomic bomb tests in the 1960s and 1970s, and again in Eastern European countries after the 1986 nuclear disaster in Chernobyl.

Researchers believe the higher number of male births, a ratio of 105 to 100, may be caused by damage to X chromosomes in sperm. The study suggests that a similar decline in female births could occur in Japan and on America's West Coast due to the recent Fukushima nuclear disaster.

Waiting for Shoulders

If we had a time machine and went back to the beginning of human birthing, I imagine we would see a common pattern of delivery—the mom would push out the baby's head. There would then be a lull in contractions. The baby's head would restitute (untwist at the neck) and then turn to face the mother's thigh (as the shoulders rotate); after this the mom would push the rest of the baby out with the next contraction. Having mom push before the next contraction is a very recent invention. The true normal and natural method of waiting for the next contraction ensures the baby's shoulders are in the usual and best position for birth—the shoulders should descend naturally and rotate with the contractions after the birth of the head. It is time we encourage this natural mechanism.

I researched this phenomenon of "waiting for shoulders" and discovered that most authors are in agreement with me. Consider the following sources:

Practical Obstetrics (1932) "When the head of the baby is born, the mucus and amniotic fluid are wiped away from the nose and mouth and the neck is freed from the encircling coils of the umbilical cord…. Haste should not be employed in the delivery of the shoulders; the latter are generally expelled by spontaneous mechanism."

Conference Chatter: The First Step

The recent Midwifery Today/Home Child Conferences in Moscow, Russia, bore much fruit. Besides the fantastic learning experience, and inspiration it had on all teachers and participants, the medical system was moved to consider the value of natural birth and homebirth. Those involved in the hospital told the midwives that Katerina Perkhova's work (as editor-in-chief of Home Child) and conferences have lead them to change their birth practices in the hospital. Here is Katerina's report of their first meeting. More are following. Pray other countries can get that much consideration and respect from Health Officials!

The movement for natural birth out-of-hospital in Russia started in the 1970s. Since then whole generations of children born at home have grown up, and so have generations of traditional midwives, who became real professionals in their field, despite the lack of acceptance in society and lack of official status for this profession in Russia.

Both parents and specialists (both traditional midwives and representatives of the state system) had to come a long way before a constructive dialogue was possible. It took more than 40 years before we at last sat down to negotiate and start working together—and it happened on June 20, 2011, in the Moscow City Duma as a part of the work of the committee on public health care.

The head OB/GYNs of Moscow's administrative districts, representatives of women's consultation clinics and maternity hospitals, as well as traditional midwives and specialists supporting the development of a professional service assisting in birth out of hospital, participated in the hearings.

Mark Kurtser, head OB/GYN of Moscow, stated that traditional midwives who assist at homebirths have a lot of very positive experience in preparation for natural birth and non-medicated pregnancy and birth care that could not but help influence the work of maternity hospitals and women's consultation clinics. He showed a video of a waterbirth that recently took place at his medical center. The heads of women's consultation clinics said during the discussions that they were upset by the fact that parents choosing homebirth consider the doctors in the women's clinics to be enemies and don't notice that they have changed a lot and are prepared to change even more, because more and more women want to have natural pregnancies and births.

Of course, it wasn't easy to sit at the same table and hear each other out respectfully, but both representatives of the state system and traditional midwives were very polite and open to dialogue. All the participants had one goal—healthy mothers and children. The suggestion made by our MAMA organization on the creation of a working committee made up of the representatives of official medicine, parents and traditional midwives was supported by all the participants. The committee has already started work to make homebirth in Russia safe, midwives professional and birth in hospital devoid of aggression.

I think that the main achievement of this meeting was that representatives of the authorities and the medical community agreed that birth is a human rights issue. They agreed that banning homebirth is a dead end in itself, which leads to the marginalization of the movement for natural birth, and that illegal homebirth is not as safe as a planned homebirth with a professional, traditional midwife.

Of course, it's still too early to speak of the legalization of traditional midwives. We have a long way to go still. But the first step has been made and may it be the start of big changes in both Russia and the world.

When the focus is on the jillion little things that can possibly go wrong, we teach fear and distrust of the body's innate ability to do its work. We need to stay in the norm and cushion students in their first year of education—with normal birth. We need to remain within the circumference of the "nutrition-plus-exercise-plus-relaxation-equals-outcome" model. If we try to teach the medical model—and especially if we teach this initially—we cannot hope to prepare midwives to advocate [for] and facilitate normal birth.

Dear Friends,

My name is Laxmi Tamang and I am one of the founding members of Nepal's first and only nurse-midwife-led independent birthing centre, as well as a founding member of the Midwifery Society of Nepal (MIDSON), established in 2007 and 2010, respectively.

Our birthing centre (http://www.apskendra.org.np/target) is struggling for sustainability because we are not donor funded. During its establishment, each of the 11 nurse members contributed $1333 to set up the birthing centre. We registered it as a social enterprise and have had very good cooperation and collaboration with the government of Nepal.

We provide free maternity care to all women who come to give birth in our centre, as well as a range of sexual and reproductive health services to urban poor families; these include family planning, antenatal care, delivery, postnatal care, immunization and STI treatment and management.

Since May of this year, Julie-Ann Dowdell, an Australian midwife, has been volunteering at the centre, and at the same time carrying out her research on the perspective of consumers, staff and others towards the birthing centre.

Getting Back into Your Body

After my trauma the last place I wanted to be trapped was in my body.

As a matter of fact, I wanted nothing to do with that physical form that was the source of so much pain I could barely contain the memory. Instead, I existed only in my head, and when that got too painful: I dissociated outside it all.

Of course, you can't go through life that way and during my recovery I sought a way to come to reconnect with my body and learn to live inside of it with trust, strength and power. I did this through dance, but that's not the only way.

Last week on YOUR LIFE AFTER TRAUMA the leaders of the yoga program at the Trauma Center of the Justice Resource Institute explained – with a whole lot of science mixed with psychology – how and why yoga can help reintegrate the mind/body connection in PTSD recovery.

Dave Emerson and Dr. Elizabeth Hopper are the co-authors of OVERCOMING TRAUMA THROUGH YOGA: Reclaiming Your Body. They said some very interesting things, including:

How yoga changes your recovery approach from top down to bottom up (I thought this was the most interesting point in the whole discussion!).
How yoga helps re-establish a sense of existence.
How the way we sense ourselves changes when trauma alters the brain.
How the physical aspects of yoga can reach emotion feelings.

Nineteen years ago I ? and my baby ? almost died. I was 29 weeks pregnant at the time. The baby was my first, and all seemed well. Then a visit to an antenatal clinic changed everything. My blood pressure was high; there was protein in my urine. I was sent to hospital; admitted immediately; and by dawn the next morning, I'd had a Caesarean section and had a tiny (2lbs 13oz) daughter, who was fighting for her life in special care.

Last year, MedEdPPD assisted Drs. Sapana Patal and Katherine Wisner in conducting a survey to explore women's perspectives about the treatment decision-making process for depression during pregnancy and after birth. Over one hundred pregnant and postpartum women completed an anonymous web-based survey regarding treatment decision making for depression. The survey data reveal that most women in this sample prefer an active collaborative role in treatment decision making for depression. If you haven't done so already, please note that on September 14-18th, Post Partum Support International, in conjunction with PSI of Washington, will be conducting their 25th Annual Conference, ""Whole Care for the Whole Family", in Seattle, Washington.
